Nurse Practitioner, Primary Care – Plainfield, New Jersey
Primary Care Partners, LLC is seeking a dedicated part-time Nurse Practitioner to join a well-established practice in Plainfield, New Jersey. This role offers the opportunity to provide care in a collaborative, patient-centered environment alongside an experienced physician and support staff. This position requires 24 hours per week, with no on-call duties.
The Nurse Practitioner will deliver comprehensive primary care to a diverse patient population, ages 18 years older and up. The Nurse Practitioner will emphasize health promotion, disease prevention, and chronic disease management, while conducting patient evaluations, developing treatment plans, and coordinating care with the broader care team.
Responsibilities:
- Conduct patient screenings and assessments
- Develop and implement individualized care plans
- Perform diagnostic tests and procedures, including I&D
- Educate patients and families on healthcare management and preventive care

Qualifications:
- Master's degree in Nursing (MSN) from an accredited program
- Current Nurse Practitioner certification and state licensure in New Jersey
- 1 year of experience is preferred, but new graduates are welcome to apply.
Hourly Range: $60-$70 per hour, dependent on experience.
