Description

Care for an established Medicare patient population in an outpatient setting with a Monday through Friday schedule, no call responsibilities, and dedicated clinical support.

Practice & Cases

• Outpatient preventive and primary care clinic
• Adult patients, primarily age 65 and older
• Manage chronic conditions including diabetes, hypertension, CHF, CAD, and hyperlipidemia
• Medical assistant, scribe, and RN support

Schedule

• Monday through Friday
• 8:00 a.m. to 5:00 p.m.
• Daytime outpatient clinic coverage
• No call responsibilities

Requirements

• Must be a Nurse Practitioner
• Board Certified in Family Medicine or Internal Medicine
• Active Illinois medical license required
• DEA required

Logistics

• Rental car and mileage reimbursement available when applicable
• Airfare and lodging are not included
