Position Highlights

• Outpatient Primary Care practice in a Rural Health Clinic setting
• Build meaningful, long-term relationships with patients and families
• Collaborate with experienced Nurse Practitioners, specialists, hospital providers, and care management teams
• Participate in population health initiatives, including:
• Annual Wellness Visits
• Chronic Disease Management
• Preventive Care Programs

Requirements

Qualifications

Required

• MD or DO degree
• Board Certified or Board Eligible in:
• Family Medicine, or
• Internal Medicine
• Licensed or eligible for licensure in Illinois
