Connected Healthcare Solutions, LLC is proud to partner with Iroquois Memorial Hospital (IMH) who is seeking a compassionate and collaborative Primary Care Physician to join our Rural Health Clinic team (MD or DO). As a Critical Access Hospital serving East Central Illinois and Northwest Indiana, IMH offers physicians the opportunity to practice broad-spectrum primary care while working closely with specialists, hospital providers, nursing homes, and community partners.
Position Highlights
- Outpatient primary care practice in a Rural Health Clinic setting
- Opportunity to build long-term relationships with patients and families
- Collaborate closely with experienced Nurse Practitioners, specialists, hospital providers, and care management teams
- Participate in population health initiatives, including Annual Wellness Visits, chronic disease management, and preventive care programs
- Opportunity for nursing home rounding.
- Access to on-site laboratory, imaging, specialty clinics, rehabilitation, and hospital services
- Supportive leadership team committed to provider engagement and reducing administrative burden
- Competitive compensation and benefits package (Base salary package starts at $325K and includes s sign on bonus and relocation package.)

Requirements
What We're Looking For:
MD or DO licensed (or eligible for licensure) in Illinois
Board Certified or Board Eligible in Family Medicine or Internal Medicine
Commitment to providing high-quality, patient-centered care
Strong communication and team collaboration skills
Interest in rural healthcare and serving underserved communities
Willingness to participate in quality improvement and population health initiatives
