- Family Nurse Practitioner
Description
The Family Nurse Practitioner provides patient care in the Church Health Medical Clinic, treating both an established patient panel as well as urgent care visits. This position educates patients and families about preventive care, medical issues, use of prescribed medical treatments, and or medications.
- Take patient history, conduct physical examinations and communicate effectively with patients and families.
- Order laboratory studies and diagnostic procedures to diagnose medical conditions.
- Prescribe necessary medication, treatment and services for quality patient care.
- Carry out therapeutic procedures as needed within the scope of primary care.
- Maintain accurate records within the EMR.
- Answer inbox messages within 48h and maintain PAQ less than 2 weeks.
- Maintain effective relationships, being mindful of any cultural differences.
- Analyze new knowledge gained from conferences, workshops, professional literature, or ‘hands-on” training and assimilate this knowledge into clinical practice.
- Review reports on individual provider progress, as well as clinic population health progress.
- Provide patient care management and pre-operative care for patients, update physicians and work with members of medical staff to allow for consultation, collaboration, or referral.
- Supervised by an agreed upon staff physician for the purposes of chart review, quality care delivery, and clinical standards and practices adopted by Church Health in accordance with the Nursing and Medical Boards of TN.

Requirements
Required Skills/Abilities
- Demonstrated passion for patient care in an underserved population.
- Understanding of quality improvement.
- Knowledge of outpatient health care procedures and counseling.
- Ability to respond to demanding and complicated patient situations.
- Flexible and adaptable in finding solutions for difficult situations.
- Knowledge and efficiency with EHR systems.
- Ability to communicate effectively both oral/written with a broad base of stakeholders.
- Demonstrated leadership skills are required in this position as the employee will be instructing, guiding, and motivating staff to perform.
- Moderate level of desktop computer skills to include the O365 productivity software suite.
Education and Experience:
- Master’s Degree: A graduate-level degree in a relevant field is required.
- Tennessee RN license is required.
- Certification as an Advanced Practice Nurse is required.
- Prescriptive Authority and DEA Licensure is required.
- 3-5 years of experience.
