Consultant Pharmacist(s) with roles and responsibilities as required by state and federal rules and regulations in our Texas area-based provider sites in Ft Worth.
Services represent the continuum of residential senior care settings (Post-Acute/Rehab, Long-Term Care, Assisted Living Facilities), and also correctional facilities located in Texas.
The position is hybrid with remote from home and on-site to provider sites.
You will provide state and federal required monthly medication regimen reviews and required oversight of pharmaceutical services for senior licensed residential long-term care nursing communities in Texas and surrounding areas and collaborate with facility leaders and staff to coordinate pharmaceutical services for the provider site.
Guide development and evaluate pharmacy procedures within the facility. Assist the facility to identify, evaluate, and address and most importantly resolve pharmacy-related concerns and issues that affect resident care, quality of life, quality of outcomes for the provider, and/or regulatory compliance. This position requires a motivation to build on existing customer relationships that promote and drive organic growth as well as drive new relationships and business opportunities with all members of the healthcare team. You will also collaborate with Long-Term Care Texas Pharmacies and their respective account management and operations team members directly to meet overall provider goals and objectives and maintain compliance with state and federal requirements.
In addition to performing Medication Regimen Reviews (MRR) and Interim Medication Regimen Reviews (TOCs, I-MRR) as outlined by regulation or contract, on-site visits and education opportunities with all levels of nursing that include auditing of medication storage, labeling and safe use with direct observations with residents and caregivers with follow up education with each occurrence and opportunity are part of your monthly roles and responsibilities. Additionally, as a member of our team, you will engage in interdisciplinary meetings involving nursing, prescribers, social services and other participants that discuss resident focused needs and outcomes related to medications, particularly those that put a resident at high-risk. Categories of medications may include psychoactive and psychotropics that are discussed in a comprehensive manner to promote safe, effective and lowest doses possible for older adults are a priority with this process, usually on a quarterly basis.
You will be involved regularly with final disposition of medications whether returns for credit to a provider's pharmacy or for destruction, regularity depending on provider site/needs and preparations.
Travel to sites in and around the Ft Worth Texas area is required. This role could possibly include an infrequent overnight trip.

Requirements
Licensed pharmacist in the state of TX, must be active & in good standing, prefer experiential training / experience in clinical practice in the care of older adults if not consultant pharmacist experience; Board Certification in Geriatrics (BCGP), completion of a fellowship or advanced training with PGY1 and/or PGY2 residency also preferred, not required. Pharmacist-Grad Interns will also be considered, provided Texas licensure expected in less than 45 days.
