Responsibilities
Member of AFM provider team, ensuring high-quality care delivery in an empathetic, efficient, and efficacious manner.
Satisfy leadership-driven clinical outcome metrics regarding patient quality, safety, as well as volume considerations.
Participate in all applicable meets.
Facilitate effective communication between patients, their families, and the healthcare team.
Leverage electronic health records (EHR) and other healthcare technology to enhance care coordination.
Collaborate with senior leadership to develop and implement strategies that align direct patient care with AFM care programs in the context of organizational goals and objectives.
Requirements
We are excited to share that Annandale Family and Internal Medicine is recruiting for an Advanced Practice Provider (APP). This role is essential and will serve as a primary factor for our patient care services and growth strategies.
The purpose of this position is to serve in alignment with our mission of serving our community as a trusted healthcare partner with a focus on individualizing patient care beyond the exam room with foci regarding prevention, wellness, and the whole person.
The APP will partner closely with the leadership team including the practice physicians, physician-owner, CFO, administrative leads, as well as our vendor leaders. This collaboration will afford the ability to translate this energy from strategy into the improvement of care delivery along the spectrum of our clinic upwards to the system level.
We are seeking a provider with a passion for Value-Based Care (VBC). Specifically, the physician will serve as a curator and a primary lever for our core care programs, such as Behavioral Health Integration, Collaborative Care Model, Chronic Care Management, Remote Patient Monitoring, and our In-Home Program designed to mitigate risk such as inpatient hospital readmissions. The provider will be able to leverage these strengths to augment the efficacy, efficiency, and impact of their patient panel, with a focus on personalized patient care that has an effect on population health.
