About the Opportunities:
Managers, Case Management are responsible for the execution and implementation of Case Management strategy and goals and the oversight of operations. The incumbent oversees the effective integration and continuous improvement of processes and systems meeting current and future business goals. Responsibilities include managing and developing a team of clinical and support staff promoting their professional growth and operational effectiveness as well as driving initiatives, and ensuring team is meeting quality standards, business goals, productivity, and achieving clinical outcomes.
Case Managers utilize the integrated (physical, behavioral health AND social drivers of health) case management model to partner with members, their providers, and caregivers to identify and address member’s barriers optimal health. Key responsibilities include engaging members, conducting relationship-based assessments, developing collaborative care plans that support improved member condition self-management skills, utilizing evidenced-based skills to promote behavior changes, ensuring care coordination, providing post-discharge follow up and delivering clinical and quality outcomes (e.g., reduce avoidable emergency department visits and readmissions and closing care gaps). Requires excellent skills in initial and sustained engagement and intervention both telephonic and digital, critical thinking, problem-solving, and motivational interviewing.
