Center of Hope, part of Saint Francis, located in Newark, DE is seeking a Primary Care- Advanced Practice Provider to support continued primary care expansion and access initiatives across the Delaware market.
Full-service Family Medicine Center providing primary care to a medically underserved population in the Newark area, with a strong focus on the Latino community. The practice emphasizes cultural component care, supported by a bilingual care team committed to delivering inclusive, patient-centered services.

PA or NP from an accredited institution
Eligible for Delaware licensure
Professional working proficiency in Spanish
Commitment to high-quality, patient-centered care
Role Responsibilities
Provide comprehensive primary care for patients across the lifespan
Assist with the diagnosis and management of acute and chronic conditions
Emphasize preventive care and population health management
Collaborate with interdisciplinary teams to optimize patient outcomes
Maintain timely and accurate documentation
Participate in quality and performance improvement initiatives
Schedule & Call
Full-time schedule (36 patient-facing hours)
