Location: Gateway Plaza, Indianapolis, IN
Position: Healthy Results Educator
Shift: 1.0 FTE Monday-Friday with two evening shifts (one until 7p & one until 8p) This position is currently hybrid requiring in office 1 time a week (subject to change)
IU Health is Indiana's largest and most comprehensive healthcare system, dedicated to improving patient outcomes through education and proactive care management. We are seeking a dedicated Healthy Results Educator to provide patient education on chronic disease management, conduct outreach, and support risk stratified populations in achieving optimal health.

Position Overview:
The Healthy Results Educator works within the wellness program and delivers telephonic patient-centered education focused on disease risk factors, prevention, medication management, and health counseling for individuals with chronic conditions. This role involves assessing patient needs, implementing education plans, evaluating outcomes, and following up to close care gaps. The Educator conducts proactive outreach, coordinates referrals, and navigates patients to appropriate resources to support health improvement efforts. The incumbent communicates effectively with risk stratified populations to promote disease management and wellness.

Key Responsibilities:
Educate patients about disease risk factors, prevention, medication, and self-management strategies.
• Assess patient needs and develop individualized education and intervention plans.
• Implement and evaluate education initiatives, ensuring goals are met.
• Follow up with enrolled participants to monitor progress and address gaps in care.
• Conduct proactive outreach and intervention with risk stratified individuals.
• Identify care gaps and communicate or facilitate referrals to appropriate services.
• Maintain documentation of education activities and patient interactions.
• Collaborate with care teams to support patient engagement and health outcomes.

Qualifications & Requirements:
• Education:
- Bachelor's Degree in Nursing or related field required.
• Licensure & Certifications:
- Active Registered Nurse (RN) license in Indiana or Nurse Licensure Compact (NLC).
- Certified Diabetes Educator preferred.
• Experience & Skills:
- 5-7 years of clinical or relevant experience preferred.
- Proficiency with Microsoft applications required.
- Ability to conduct outreach and support patient engagement.
- Strong communication and educational skills.
- 5-7 years of diabetes educator experience strongly preferred.
• Travel:
- Minimal travel within the state required.
