Licensed Vocational Nurse (LVN) - Skilled Nursing *Baylor Scott & White Health | Continuing Care Hospital (CCH)* *Temple, TX | Day Shift* Baylor Scott & White Health is seeking a compassionate and motivated *Licensed Vocational Nurse (LVN)* to join our *Skilled Nursing team* at the *Continuing Care Hospital (CCH)* in Temple. This is a great opportunity to work with medically complex patients in a supportive, team‑oriented environment while continuing to grow your nursing skills. About Continuing Care Hospital (CCH) Continuing Care Hospital provides specialized care for medically complex patients across *critical care, acute care, and skilled nursing settings*. Our patients often require extended care, with an *average length of stay of approximately 25 days*, allowing nurses to build meaningful relationships and expand clinical expertise. Position Details * *Shift:* Day Shift, 7A-7P * *Status:* Full‑time, (3) 12-hours shifts * *Patient Population:* Medically complex patients requiring special and extended care * *Environment:* Skilled Nursing within a Continuing Care Hospital setting Key Responsibilities * Provide direct patient care to medically complex patients in a skilled nursing environment * Collaborate with the interdisciplinary care team to support patient recovery and outcomes * Monitor patient conditions and report changes promptly * Administer medications and treatments as ordered * Maintain accurate and timely documentation * Promote teamwork, professionalism, and a positive patient experience Qualifications * Graduate of an accredited *Nursing program* * *1 year of LVN experience required* * *Med/Surg or Skilled Nursing (SNF) experience highly preferred* * Current *Texas LVN License* (Vocational Nursing Practitioner's License) * *BLS certification required* (may be obtained within 30 days of hire) * Self‑motivated, team‑oriented, and patient‑focused approach to care About Us Here at Baylor Scott & White Health we promote the well-being of all individuals, families, and communities. Job Summary As a Licensed Vocational Nurse, you play a crucial role in a healthcare team. You evaluate patients, provide care, and participate in care planning, intervention, and evaluation. Focus on precision, vigilance, and patient care. Note and report any changes in patients' conditions to your Registered Nurse supervisor. Follow Texas Nursing Practice Act regulations. Essential Functions of the Role * Conduct thorough assessments using clinical reasoning and best practices following the LN Scope of Practice. Your findings will help the Registered Nurse create care plans and set goals. * Participate in evaluating nursing practice effectiveness and quality, by reviewing and examining specific indicators in nursing. * Create a caring, safe, healing, supportive, therapeutic environment. * Manage pain, promote comfort, minimize suffering. * Facilitate education for patients, families, nurses, and other healthcare team members, as well as the community. * Work with others to implement a care plan under RN/provider direction, including team members and community. * Employ strategies and utilize available resources to solve problems for patients, their families, and staff. Key Success Factors * A thorough knowledge of nursing and patient care standards and procedures. * Knowledge of rules and regulations established by certifying bodies affects nursing care delivery in the field. * Familiarity with medical terms, health promotion methods, risk reduction techniques, disease prevention and management practices, medications and their potential effects. * Excellent communication skills, enabling the clear expression of thoughts both orally and in writing. * Superb interpersonal skills to foster interactions with a diverse range of people. * Ability to think critically and solve problems effectively. * Capacity to identify and communicate any changes in a patient's medical condition to other nurses and healthcare providers. * The ability to offer good care to patients of all ages, while honoring each person's unique experiences. * Basic computer literacy, encompassing Microsoft Office, information security protocols, scheduling and payroll systems, electronic medical record maintenance, and email usage.
