Summary

The Advanced Practice Nurse (APN) Certified Family Nurse Practitioner (FNP) practices in Behavioral Health Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission.

Duties

• Duties of the APN-FNP include - but are not limited to: Manages a panel of patients with acute and chronic health conditions - focusing on health promotion and disease prevention using evidence-based practice
• Develops and implements individualized care plans to achieve optimal health outcomes
• Orders and interprets labs - tests - and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services as needed
• Assesses and adjusts care plans to ensure optimal patient outcomes
• Provides education to patients and their families regarding health conditions - treatments - and preventive care
• Demonstrates leadership and works collaboratively with interdisciplinary teams
• Leads and participates in performance improvement initiatives and quality improvement projects
